Officer Declaration Form

This form must be digitally signed by all group officers. A printed receipt must be turned in after the form is completed. If you have a registration or officer update in progress you will also be able to see what you need to do to complete the process.

Access the Form

You will be required to log into your University of Minnesota account if you are not already logged in. If you do not have a U of M Internet Account, you should have received a token for the Non-University Advisors form below.
